Lung Cancer Presenting as Advanced Gastric Carcinoma

Case details from Feb 2025: 63-year-old male, a former smoker, with initial presentation suggestive of an advanced GI malignancy: Weight loss, poor appetite and eventually findings of a large gastric lesion as well as 12 mm pulmonary nodule.

Initial biopsy was suggestive of a gastric adenocarcinoma, PDL and Her-2 positive.

Additional staining revealed TTF positivity, and CancerTYPE ID testing suggested his gastric lesion to be a metastasis from lung primary.

The patient is now on Pembrolizumab/Pemetrexed and Carboplatin with evidence of response.
